WE ARE NOT CURRENTLY ACCEPTING NEW ADULT PATIENTS. New pediatric and returning patients can schedule appointments online (strongly preferred), by texting 503-662-7226 (second best), messaging us through this site, or by phone. Please read the visit descriptions and see our detailed self-pay price list below. Clinic visits are available on Mondays, Tuesdays, Wednesdays, and Fridays from 10 AM-7 PM. We are currently in-network for many plans with Cigna, Blue Cross Blue Shield, Pacific Source, Moda, Aetna, and UnitedHealthcare. Unfortunately, we do not take the Oregon Health Plan. Please check with your insurance company to see if we are in-network for your specific plan. If not using insurance, all fees must be paid on the day of service using cash, check, card, or app. Appointments cancelled less than 24hrs in advance may be subject to a $75 fee. If you can't afford the care you need, please contact us before scheduling to discuss the financial aid options available to you.
We donate 3% of all proceeds to our Patient Support Fund. This money pays for needed medical services when a patient is unable to afford them. Please consider donating!
If you need a Primary Care Provider (PCP), Dr. Teresa Bair would be happy to help. As your PCP she will learn about your medical history, and provide comprehensive preventative and problem-focused care tailored specifically you. Her appointments are much longer than you'll find at other clinics, allowing plenty of time for listening to your concerns, answering questions, discussing treatment options, and patient education. She will also help coordinate care with any outside services or specialists you may need. We only see patients 6 years and older for Primary Care, as we do not offer immunizations on-site. You can choose either a clinic or video visit, though a physical exam is recommended within 1-2 months of establishing care.
Single-isssue visits for all ages. Same-day clinic and video visits are often available, and most patients can be seen in 1-3 days for acute conditions. Appointments can last up to 60 minutes. Common reasons for these visits include acute illness or injury, cancer screenings, urinary tract infections, skin conditions, sexual health concerns, and many more, These appointments are not as comprehensive as Primary Care visits, and ongoing care coordination will not be provided in most cases.
We do not see patients for workers' compensation or motor vehicle claims.
Osteopathic Manipulative Treatment (OMT) is a hands-on method of diagnosis and treatment for a wide variety of health conditions, and to improve overall wellness. Though these treatments can appear similar to chiropractic or massage therapy, they are performed by a fully-licensed Osteopathic Physician who takes your entire medical picture into account. OMT appointments are available to all ages, including pregnant patients and infants. Common conditions treated are headaches, back pain, muscle spasms, digestive discomfort, congestion, anxiety, infant feeding problems, and much more. Patients typically wear loose or athletic clothing, and treatments can be focused on one area or broadened to include the whole body.
We offer an affordable care plan especially well suited for those with little-to-no health insurance or who need regular visits. For $100 per month our membership plan includes unlimited clinic visits, prescription orders, referrals, and recommended in-office labs. This does not include outside labs, medications, or off-site services. Payments will be automatically withdrawn each month, and there is a 6-month minimum membership to start.
At this time we are unable to provide the following services on-site:
Disability paperwork will only be completed for established Primary Care patients.
If you want us to bill your insurance, great! Please check with your insurer to make sure we're in-network, and bring your physical card to every visit. We will bill them directly, using industry standard fee-for-services pricing. For those not using insurance, the estimated self-pay prices below are significantly discounted, and must be paid at the time of service. Please let us know if you ever end up with a bill that is more than you can afford, or that seems unexpectedly high.
